After trying more than 15 solutions to fix this game to run in Win10, i got it running by installing the following:
4gb patch
nvse
nvac
nv stutter remover.

ALTHOUGH it says its running but i have no display of the game. sometimes two instances of fnv are running but again i dont have display of the game.

my font scaling is on 100% and my H/W are 1920/1080.

i tried using the d3d9.dll and nothing.

i ran the game from steam=nothing, from .exe=nothing, from nexus mod manager=nothing, from nvse launcher=nothing.

I had this exact same problem. I can't remember exactly what fixed it, but I think it was this
""C:\Program Files (x86)\Steam\steamapps\common\Fallout New Vegas\FNV4GB.exe" -laaexe .\FalloutNV4GB.exe"

hey guys i ran into this problem as well. This is how i fixed it.
1.On pc start up, go to steam
2.go to library, then fallout newvegas
3.right-click on fallout new vegas, go to properties, local files, verify game cache
4. after this id done, run the falloutnv patch(from nexusmods)

after verifying cache and then patching the game works fine. until i restart my pc. then i have to repeat these steps. to me this isn't that painful of a work around. but then again i did grow up in an era when ppl had to blow in the game and fight with it to get it to work. but this points to a steam update causing this drama.

Does task manager show two instances running? Are you using a Mod manager to launch New Vegas? Have you tried using a shortcut executable with no parameters (not running with NVSE) Does this occur with a unpatched exe?

I recommend you read the entire "FNV General Mod Use Advice" article to understand the differences between this game and others you may have experience with; especially if this is your first attempt to mod FNV or it's been more than a year since you last did so.

I've stumbled upon a weird issue that I haven't encountered or read about before. I'm running Windows 8.1 and a fresh install of Fallout New Vegas with no mods so far. I'm trying to get Mod Organizer and the 4GB exe to work together, but despite following all of the advice, tips and workarounds it's still not happening. Right now I can launch the 4GB exe through Mod Organizer. The game starts, I can load a save and 'play'. The problem is that all of the text has disappeared or is invisible. I can only navigate the main menu by memory, the ingame popups regarding the DLC is missing the text as well.

This issue does not happen if I start the game on its own with the normal exe.
This issue does not happen if I start the 4GB exe on its own.

It only happens if I start the either the normal or the 4GB exe through Mod Organizer.

NVSE is being loaded, it doesn't matter if I'm using NVSE stable or beta.
I'm using NVAC, without it the game crashes before arriving at the main menu. NVAC seems to be working, the log is showing several handled errors. (Pastebin)
Yes, I've added '-laaexe .\FalloutMO.exe' to the launch arguments, that helped me get around the access is denied error but produced this text error.
I did try compatability modes, I've tried starting MO, the 4GB or both as administrator. No dice.

I've arrived at a point where I really don't know what it could be or what I could try next. One thing I do know, however, is that I can't go back to the pre-MO modding, so I'm turning to you helpful people and hope that someone here has an idea.

I deleted all of the fallout .inis as well in case some of my text/font settings were messed up, but that didn't work either.

I keep getting "ERROR. There is already an instance of Fallout New Vegas running." When launching from Mod Organizer.

I followed the step guide, though i didn't download many mods.

Most of the time, when i run the game in Mod Organizer, the launcher will come up, i'll click play, and sometimes the game will launch and play normally. The majority of the time however, it does not and i end up with the warning that the game is already running.

When this happens i can not see any evidence of the game running exept for when i check task manager and click 'more details' where i can find fallout and fallout 4gb running. I can end these and try again, but mostly this will repeat.

My load order is:

YUP - Base Game + All DLC .esm

Project Nevada - Core.esm

Project Nevada - Equipment.esm

Project Nevada - Rebalance.esp

Project Nevada - Cyberware.esp

Project Nevada - Extra Options.esm

Project Nevada - Rebalance Complete.esp

Project Nevada - Cyberware Additions.esp

Project Nevada - All DLC .esp

The Mod Configuration Menu.esp

Bashed Patch, 0.esp

I would greatly appreciate any help, as i am quite new to modding and don't know how to fix this. This problem has been driving me crazy :(
